A friend of mine can't control his bladder anymore and claims it hurts to urinate now?

My friend is on disability and went to the doctors a week ago or so, but that was only when it was leaking. He now has absolutely no control of his bladder, and is telling me it hurts when it happens at first it was mild pain but now very severe. He’s afraid to go to doctor again cause of his very strict medical plan, can anyone tell me what he might have and if he should get to the doctor asap?
He definitely needs to see a doctor. He could have something as serious as prostate cancer, or an std.
he needs to get checked. but maybe bladder infection or std or cancer. goodluck =)
